docs: update keyboard shortcuts and config documentation

Updates README with Ctrl+ only shortcuts (removes Cmd/Ctrl variants), simplifies config reference to point to docs/CONFIG.md, adds JSON Schema feature to CHANGELOG, and notes BREAKING change for keyboard shortcuts migration.
