#2 WIP: fix(modes): reset terminal title on /clear

/clear vidait le transcript mais gardait l'ancien titre de terminal (OSC 0), et aucun nouveau titre n'était généré pour la conversation suivante.

  • Ajout de resetSessionTitleLocked() dans session_title.go : annule la génération de titre en cours, remet le titre neutre « zut », réarme la génération au prochain prompt réel.
  • Utilisée par /clear, le reset de session côté host (/cd) et resetSessionTitleForFreshBranch (suppression du code dupliqué).
  • Le reset host efface aussi firstRealPrompt, comme les autres resets.

Tests : nouveau TestClearResetsTerminalTitleAndRegeneratesOnNextPrompt (échoue sans le fix). gofmt, go vet, go test ./..., go test -race -count=10 -run 'Title|Clear' OK. Wrap-up review : aucun finding.
